ABSTRACT

ObjectiveTo explore the effects of body weight and waist circumference on static and dynamic balance function in overweight and obese adults. MethodsParticipants (n=103) were selected by cluster random sampling from the Hypoglycemic Weight Loss Clinic of the Endocrinology Department of the Third Affiliated Hospital of Sun Yat-sen University. All participants were assessed for basic data collection, biochemical test, body weight, waist circumference and height measurement, static balance function assessed by balance error scoring system (BESS) and dynamic balance function assessed by functional reach test (FRT). The BESS score and FRT distance of all participants were compared among groups according to different BMI grades and WC grades. Multivariate linear regression was used to analyze the influencing factors of participants' dynamic and static balance functions. Results① With the increase of BMI grading, the BESS score showed an upward trend, and the difference between groups was statistically significant (P = 0.004). The BESS score of normal WC patients was lower than that of central obesity patients (P < 0.001), which indicated that compared with normal BMI, overweight and obese people had poor dynamic and static balance ability; ② With the BMI grading, the FRT distance increased, showing a downward trend (P < 0.001). The FRT distance in normal WC patients was significantly higher than that in central obesity patients (P < 0.001), which indicated that the static and dynamic balance ability of central obesity patients was worse than that of normal WC patients; ③ In BMI overweight group, the FRT distance decreased significantly with the increase of WC (P = 0.02). The results showed that under the condition of no difference in BMI, Compared with normal WC, the dynamic and static balance ability of central obese patients was worse; ④ The influence of BESS score on BMI(B=4.12,P =0.027, 95% CI=0.48-7.75)and WC(B = 3.47,P = 0.046, 95% CI = 0.07 - 6.88)was significant. The influence of FRT distance on BMI(B = -5.68,P = 0.001, 95% CI = -8.95 - 2.41)and WC(B = -4.71,P = 0.003, 95% CI = -7.83 to -1.61)was significant, which indicated that the static and dynamic balance ability of obese people was worse with the increase of BMI, and the ability of dynamic and static balance of central obesity was worse than that of normal WC. ConclusionWaist circumference is an independent factor affecting the dynamic and static balance function of overweight and obese people. Under similar BMI, the dynamic balance function of central obese people is worse than that of people with normal waist circumference, leading to higher risk of falling.

ABSTRACT

ObjectiveTo study the possible correlation between serum osteoprotegerin (OPG)/soluble receptor activator of the nuclear factor κB ligand (sRANKL) levels and the left ventricular diastolic dysfunction (LADD) in patients with type 2 diabetes mellitus (T2DM). MethodsTotally 68 T2DM patients and 37 healthy controls were selected. Serum OPG and sRANKL were determined by solid-phase enzyme-linked immunosorbent assay (ELISA). The left ventricular diastolic function of T2DM patients was measured by transthoracic echocardiography, where E/A < 1 were regarded as LVDD. T2DM patients were further divided into two subgroups according to E/A ratio (E/A≥1.0 and E/A<1). Spearman correlation analysis, logistic regression and ROC curves were used to assess the possible correlation between serum OPG/sRANKL and LADD in T2DM patients. ResultsCompared with the healthy controls, serum OPG level in T2DM patients was higher with statistically significant difference (P <0.01), while serum sRANKL level was lower without statistically significant difference (P =0.32). T2DM patients with E/A<1 had significantly higher OPG level and lower sRANKL level than those with E/A≥1(P <0.01) in subgroup analysis. Spearman correlation analysis showed serum OPG level was negatively correlated with E/A ratio, while sRANKL was positively related with E/A ratio. In single factor logistic regression analyses, serum OPG [OR (95% CI)=1.068 (1.031, 1.106), P<0.001] and sRANKL [OR (95% CI)=0.976 (0.959, 0.992), P=0.003] were significant correlation with LVDD in T2DM patients. ROC curve analysis showed that the sensitivity and specificity of combined OPG and sRANKL in diagnosing T2DM patients LADD were 78.13% and 88.3%, respectively (area under the curve: 0.857; 95% CI=(0.768, 0.946); P<0.001). ConclusionsThe elevated OPG and decreased sRANKL levels may be associated with LADD in T2DM patients.

ABSTRACT

Objective To investigate the prevalence and genetic variation of Theileria in yellow cattle in Xiangxi Autonomous Prefecture of Hunan Province. Methods A total of 184 blood specimens were collected from Fenghuang, Huanyuan and Baojing counties of Xiangxi Autonomous Prefecture during the period from August 2018 through August 2019, and were detect using PCR assay with the specific 18S ribosomal rRNA (18S rRNA) gene targeting Theileria. The gene sequences of positive specimens were aligned with the sequences recorded in GenBank, and a phylogenetic tree was created with Plasmodium ovale 18S rRNA as an outgroup. Results A total of 143 blood samples were positive for Theileria, with a mean detection rate of 77.7%. Theileria was prevalent in the blood samples from yellow cattle in all three counties, with detection rates of 85.0% in Fenghuang County, 88.3% in Huayuan County and 61.0% in Baojing County, respectively. There was no significant difference in the detection rate of Theileria between Xiangxi yellow cattle and normal yellow cattle (77.2% vs. 79.5%; χ2 = 0.08, P > 0.05), while the detection of Theileria was significantly lower in the housed yellow cattle than in free-range cattle (68.9% vs. 89.7%; χ2 = 22.36, P < 0.01). A total of 18 PCR positive samples were randomly selected for sequencing and analysis, and all samples showed more than 99.0% homology with T. luwenshuni isolates. Phylogenetic analysis showed that the 18 positive samples were clustered into the same branch with T. luwenshuni, but were far away from other isolates. Conclusions The prevalence of Theileria is high in yellow cattle from Xiangxi Autonomous Prefecture of Hunan Province, and T. luwenshuni may be the dominant parasite species.

ABSTRACT

A new fungus, Ophiocordyceps alboperitheciata, parasitic on the larva of Noctuidae (Lepidoptera) was identified from a survey of entomopathogenic fungi in Kunming Wild Duck Forest Park, Yunnan Province, China. It can be primarily distinguished from relatives by its longer fertile parts, sterile tips, superficial perithecia, narrower asci, and smaller septa of ascospores. As revealed from phylogenetic analyses inferred from nrSSU, nrLSU, tef-1α, rpb1, and rpb2 sequence data, O. alboperitheciata belongs to the Hirsutella citriformis clade in the genus Ophiocordyceps of Ophiocordycipitaceae, and forms a separated clade from other related species. The uniqueness of the taxon is significantly evidenced by both molecular phylogeny and morphology. Furthermore, the interspecific relationships in the H. citriformis clade are discussed.

ABSTRACT

Coronary artery disease (CAD) remains a leading cause of morbidity and mortality in the world. Although the comprehensive control of cardiovascular disease risk factors has achieved remarkable progress in recent years, the incidence of cardiovascular events is still high after the control of traditional risk factors such as low density lipoprotein cholesterol, blood pressure and blood glucose, collectively referred to as cardiovascular residual risk. Inflammation is a central driver of atherosclerosis and the ultimate rupture of plaque, as well as an important cause of residual cardiovascular risk. Therefore, this article reviews the formation, assessment and treatment of residual inflammatory cardiovascular risk.

ABSTRACT

OBJECTIVE@#To explore the epidemic situation of cannabis use among drug users with compulsory detained detoxification treatment in China.@*METHODS@#Using the data from the Drug Abuse Population Estimation in the Key Cities of the Ministry of Public Security, we analyzed the sociodemographic characteristics and substance use of cannabis abusers with compulsory detained detoxification treatment in 55 provincial capital cities and key cities of China. Chi-square test, Fisher exact test and Kruskal-Wallis rank sum test were used to compare the prevalence of cannabis, heroin, synthetic and mixed drug use among patients with detoxification treatment, as well as the differences in polydrug use and areas among cannabis users.@*RESULTS@#In the study, 25 366 drug users with compulsory detained detoxification treatment were recruited, of whom 2.2% (546/25 366) used cannabis in the previous year before the treatment. The proportion of males was 83.5%, and the proportion of ethnic minorities was 41.0%. Those who received junior high school education or above accounted for 30.8%, and the unemployed accounted for 44.1%. The average age was (33.3±8.2) years, the average age of beginning drug use was (24.8±7.7) years, and the average duration between the first drug abuse and first detoxification treatment was (5.4±4.6) years. The prevalence of cannabis use was higher among those drug users who were 35-year-old and younger, ethnic minorities, employees and residents in Xinjiang. Of the cannabis users, 91.4% used polydrug, 13.6% combined with heroin alone, 42.1% combined with synthetic drugs alone and 35.7% combined with both of heroin and synthetic drugs. Of the cannabis users, 49.6% came from 3 regions: Xinjiang Uygur Autonomous Region, Jiangsu Province and Shanghai City. The cannabis users in Xinjiang had a high proportion of ethnic minorities who received junior high school education and below. Moreover, 79.6% of them combined cannabis use with heroin. The cannabis users in Jiangsu, Zhejiang and Shanghai areas had a higher proportion of ethnic Han who received better education (high school and above). Moreover, 92.7% of them combined cannabis use with methamphe-tamine.@*CONCLUSION@#The prevalence of cannabis use among the population with compulsory detained detoxification treatment is higher than that among drug users under surveillance, but there are obvious regional cluster effect and high possibility of polydrug abuse. Thus, it's important to strengthen the monitoring of cannabis use, to increase the control of cannabis and to formulate China's anti-cannabis policy among different population.

ABSTRACT

Itching is an unpleasant sensation that can cause physical and psychological discomfort. It often occurs in a variety of diseases. In addition to the common chemical medicines, the application of traditional Chinese medicines(TCMs) have attracted much attention for itch. As one of the classic antipruritic formula, Xiaofengsan is widely used by the later generations of doctors based on the edition of Orthodox Manual of External Diseases(Wài Kē Zhèng Zōng). It can be used to remove wind, dehumidify, clear heat and nourish blood by taking into account the complicated pathogenesis of skin diseases. In modern clinics, Xiaofengsan is commonly used for the treatment of rubella, eczema, acute and chronic urticaria, contact dermatitis, allergic dermatitis, psoriasis and other types of dermatitis with remarkable antipruritic effect. Its mechanism is related to its anti-allergic reaction, anti-allergy, anti-inflammatory, anti-histamine,immune regulation etc. This article reviews the research literature of Xiaofengsan and its addition and subtraction in modern pharmacological experimental research and clinical treatment in the past 15 years, and analyzes the current research situation of Xiaofengsan in the topic of antipruritic effect. The focus is on the compatibility characteristics of formula and the component study of single herb after being decomposed of Xiaofengsan, and the main material basis of antipruritic effect is summarized. It was found that Schizonepetae Herba, Saposhnikoviae Radix, Arctii Fructus, Cicadae Periostracum and Sophorae Flavescentis Radix were the main medicines for its antipruritic effect, which was helpful to expand the clinical value of Xiaofengsan and deepen its mechanisms of anti-itching.

ABSTRACT

Objective@#Symptomatic intracranial hemorrhage (sICH) is one of the severe complications of ischemic stroke thrombolysis. Several prognostic scales have been developed to predict the risk of sICH. The performance of seven scales was compared in a single center cohort.@*Methods@#Data of patients with consecutive ischemic stroke who received 0.9 mg/kg intravenous recombinant tissue plasminogen activator (rt-PA) thrombolysis within 4.5 h time window from stroke onset were collected. Seven scales that can provide an estimate of risk of sICH were identified and evaluated: Hemorrhage After Thrombolysis (HAT), blood Sugar, Early infarct signs, (hyper) Dense cerebral artery sign, Age, National Institutes of Health (NIH) Stroke Scale (SEDAN), Stroke Prognostication using Age and NIH Stroke Scale (SPAN)-100, Safe Implementation of Thrombolysis in Stroke (SITS), Total Health Risks In Vascular Events (THRIVE), Glucose at presentation, Race (Asia), Age, Sex (male), systolic blood Pressure at presentation, and Severity of stroke at presentation (NIH Stroke Scale; GRASPS) and Multicenter Stroke Survey (MSS). The area under the receiver operating characteristic curve (AUROC) was calculated and Logistic regression and the Hosmer-Lemeshow test were also performed.@*Results@#The current study included 293 patients, of whom 7.85% (23/293) had sICH by National Institute of Neurological Disorders and Stroke (SICHNINDS), 5.46% (16/293) by Europe Cooperative Acute Stroke Study Ⅱ (SICHECASSⅡ) and 4.44% (13/293) by Safe Implementation of Thrombolysis in Stroke (SICHSITS) criteria. SEDAN had the highest AUROC for predicting sICH: sICHNINDS: AUROC=0.843, OR=3.167, 95%CI 2.106-4.762, P<0.01; sICHECASSⅡ: AUROC=0.797, OR=2.509, 95%CI 1.652-3.812, P<0.01; sICHSITS: AUROC=0.784, OR=2.172, 95%CI 1.405-3.357, P<0.01. And SPAN-100 had the lowest AUROC among all the seven scales and was only associated with risk of SICHNINDS in regression analysis. Furthermore, when sub-grouped the cohort into anterior circulation infarction and posterior circulation infarction, regression analysis suggested that all the seven scales were however not associated with sICH risk in patients with posterior circulation infarction.@*Conclusions@#SEDAN constantly had the highest predictive power, SPAN-100 had the worst. The seven scales studied could not predict sICH in posterior circulation infarction.

ABSTRACT

Objective Programmed cell death ligand 1 (PD-L1) plays an important role in the treatment of malignant tumors. Thestudy explores the relationship between PD-L1 status and breast cancer radiotherapy and clinical prognosis. Methods Using the Cancer Genome Atlas (TCGA) database analyze the relationshipof PD-L1 expression, radiotherapy and prognosis in breast cancer. 1007 patients were included in the statistical analysis. Based on the median value of CD274 mRNA expression levels in the database, the total sample was divided into a PD-L1 -high state group (503 cases) and a PD-L1-low state group (504 cases). Survival analysis was carried out for patients in the high and low state group with and without radiotherapyrespectively. Results The univariate analysis showed that the overall survival rate was improved in patients with PD-L1high status compared with patients with PD-L1-low status. The difference was statistically significant(P=0.047). InPD-L1-high status group, univariate analysis and multivariate analysis showed no significant difference in the overall survival rate between patients with radiotherapy and without radiotherapy, and the P values were 0.352 and 0.242 respectively. However, the overall survival rate of radiotherapy patients with PD-L1-low status group was significantly improved (HR0.550, 95%CI:0.335~0.904, P=0.017), and there was still statistical significance after multivariate factors adjustment (HR0.415, 95%CI:0.194~0.872, P=0.025). Chi-square analysis showed that PD-L1 status was associated with ER status (χ2=18.44,P<0.001), HER2 status (χ2=7.943,P=0.047) and molecular typing (χ2=14.52,P=0.006), but not with age, histological type, surgical treatment, postoperative tissue margin status and PR status. Conclusion The PD-L1 status was associated with the radiosensitivity of breast cancer. The overall survival rate of postoperative radiotherapy in patients with PD-L1⁃low expression was significantly improved. It is speculated that PD-L1 may be a radiosensitivity gene for breast cancer.

ABSTRACT

Mast cells are widely distributed in various parts of the body, especially in the mucosal surface between the body and the external environment. Mast cell is one of the important immune cells and plays important roles in innate immunity, adaptive immunity and immune regulation. Previous researches have shown that excessive activation of mast cells is closely related to the development of allergic and inflammatory diseases such as asthma, allergic rhinitis, food allergies, acute and chronic itching. Mast cells infiltrate into the inflammation site and release various allergic mediators during the occurrence and development of these diseases. Therefore, termination of mast cell activation can be one of the effective methods for the treatment of allergic and inflammatory diseases, and receptors related to mast cell activation are potential targets for the development of anti-allergic drugs. There are many receptors related to mast cell activation, and the effects mediated by different receptors varied from each other. In the recent years, new mast cell receptors are being discovered, but there are not many literatures discussing the possible functions of these newly discovered receptors. This review aims to summarize the receptors involved in mast cell activation and classify related receptors according to their effects.

ABSTRACT

Functional near-infrared spectroscopy (fNIRS) is a relatively new technique that can measure the hemoglobin changes in brain tissue. Depending on the absorption for different wavelength, relative changes in the concentration of hemoglobin in brain can be measured, obtaining the hemodynamic characteristics of brain to understand the brain functional state in real-time. Compared with other neuroimaging techniques, it has some advantages and has been used in psychiatry rapidly. Schizophrenia is one of the most common mental disorders, researches on its pathogenesis have focused on collection of structural and functional alteration in brain. Existing techniques can provide only indirect evidence, while fNIRS can perform brain function monitoring in real-time. A number of studies have suggested that fNIRS signal could be used as a biomarker for schizophrenia identification and adjuvant diagnosis. This paper reviewed the research progress in the application of fNIRS in schizophrenia.

ABSTRACT

OBJECTIVE@#This work aimed to compare the effect of retention and preventive caries of the pit and fissure sealant on primary teeth by using glass ionomer cements (GIC) with or without acid etching technique.@*METHODS@#In this clinical trail, 100 children aged 3-4 years old with a split-mouth design were equally divided into two groups (the left second upper and lower mandibular primary molars were present in group A, and the right second upper and lower mandibular primary molars were present in group B; each group had 200 molars). All participants were placed in pit and fissure sealant with Ketac® Molar Easymix and seated with atraumatic restorative treatmen press-finger technique in the two groups. Group A was subjected to acid etching (40 s, 35% H₃PO₄), while group B were not subjected to acid etching. GIC preservation rate, caries rate, and decayed teeth (dt) index were observed after 12 months.@*RESULTS@#After a 12-month follow-up period, the completely lost, partially losing, and losing rates of GIC in 86 children (172 second primary molars) were 83.14%, 9.88%, 6.98% in group A and 62.79%, 20.35%, and 16.86% in group B, respectively. The differences between the two groups were insignificant (P<0.05). The caries rates of groups A and B were 8.14% and 16.86%, and their dt indices were 0.08±0.31 and 1.17±0.46, respectively, thereby indicating significant difference between the two groups (P<0.05).@*CONCLUSIONS@#Using acid etching technology, the retention rate of the pit and fissure sealant increased, while its losing rate decreased. The retention effect of the GIC was improved, and the dental caries prevention effect was enhanced. Sealant processing was a self-curing procedure that sets without the external energy. Hence, this procedure is suitable for the preventive caries of primary teeth in children.

ABSTRACT

BACKGROUND: Primary injuries of the spinal cord are irreversible in the pathoplhysiological process. Most of studies have focused on prevention against secondary injuries, by reducing neuronal apoptosis and necrosis as well as decreasing damage area, which provides favorable environment for axonal regeneration, blocks secondary injury and promotes neural regeneration after spinal cord injury.OBJECTIVE: To review the recent advances in the signaling pathway related to secondary injuries after spinal cord injury, and to clarify the related signaling pathways, thereby providing theoretical basis for the treatment of spinal cord injury.METHODS: PubMed, EMBASE, Medline, CNKI, CJFD and Wanfang databases were retrieved for the articles addressing the signaling pathways in spinal cord injury published between 2006 to 2016. The keywords were "spinal cord injury, signaling pathway" in English and Chinese, respectively. The signaling pathways in secondary injuries after spinal cord injury were summarized.RESULTS AND CONCLUSION: Mitogen activated protein kinase, nuclear factor of kappa B, PI3K/PKB (Akt), Janus kinase/signal transducer and activator of transcription, and Wnt signaling pathways are mainly involved in secondary injuries after spinal cord injury. All these pathways play a significant role in the development and repair of spinal cord injury, and future investigation on them is warranted.

ABSTRACT

<p><b>BACKGROUND</b>Several studies using functional magnetic resonance imaging (fMRI) and positron emission tomography (PET) have indicated that cognitive remediation therapy (CRT) might improve cognitive function by changing brain activations in patients with schizophrenia. However, the results were not consistent in these changed brain areas in different studies. The present activation likelihood estimation (ALE) meta-analysis was conducted to investigate whether cognitive function change was accompanied by the brain activation changes, and where the main areas most related to these changes were in schizophrenia patients after CRT. Analyses of whole-brain studies and whole-brain + region of interest (ROI) studies were compared to explore the effect of the different methodologies on the results.</p><p><b>METHODS</b>A computerized systematic search was conducted to collect fMRI and PET studies on brain activation changes in schizophrenia patients from pre- to post-CRT. Nine studies using fMRI techniques were included in the meta-analysis. Ginger ALE 2.3.1 was used to perform meta-analysis across these imaging studies.</p><p><b>RESULTS</b>The main areas with increased brain activation were in frontal and parietal lobe, including left medial frontal gyrus, left inferior frontal gyrus, right middle frontal gyrus, right postcentral gyrus, and inferior parietal lobule in patients after CRT, yet no decreased brain activation was found. Although similar increased activation brain areas were identified in ALE with or without ROI studies, analysis including ROI studies had a higher ALE value.</p><p><b>CONCLUSIONS</b>The current findings suggest that CRT might improve the cognition of schizophrenia patients by increasing activations of the frontal and parietal lobe. In addition, it might provide more evidence to confirm results by including ROI studies in ALE meta-analysis.</p>

ABSTRACT

Objective To explore the prevalence and risk factors of stroke in a population over 40 years in a community in Shenzhen, China. Methods The subjects investigated were a population ≥40 years old in this cross-sectional study. Cluster sampling method was used to conduct the unified questionnaires, physical examination, and laboratory examination in the community residents, and then the survey data were used for online entry analysis. Results A total of 5 308 community residents were screened, and 160 experienced stroke. The crude prevalence of stroke was 3 014. 32/100 000. The prevalence of stroke increased with age, and that in males was significantly higher than that in females (3 721. 37/100 000 vs. 2 552. 93/100 000; χ2 = 5. 923, P = 0. 015). There were significant differences in the proportions of males, obvious overweight, hypertension, diabetes mellitus, atrial fibrillation, lack of physical activity, and family history of stroke, as well as age, systolic blood pressure, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, and fasting glucose level between the stroke population and the non-stroke population (all P 80 years) in both males and females. The proportions of diabetes (P < 0. 001) and atrial fibrillation (P < 0. 001) in males, and obvious overweight (P = 0. 001) in females were gradually increased with age, and the proportion of smoking in males weas gradually reduced with age. The proportions of hypertension (P < 0. 001) and obvious overweight (P < 0. 001) in males were significantly higher than those in females at the age of 40 to 49 years. The proportions of hypertension (P < 0. 001), diabetes (P < 0. 001) and obvious overweight (P < 0. 001) in males were significantly higher than those in females at the age of 50 to 59 years. The proportion of hypertension in males was significantly higher than that in females at the age of 60 to 69 years (P = 0. 039). The proportions of hypertension (P = 0. 016), atrial fibrillation (P = 0. 028) and hyperlipidemia (P = 0. 023) in females were significantly higher than those in males at the age of 70 to 79 years. The proportion of obvious overweight in females was significantly higher than that in males at the age of ≥80 years (P =0. 001). Conclusions The crude prevalence of stroke is higher among the community residents. The the levels of exposure to stroke risk factors including hypertension, diabetes and atrial fibrillation are higher. It may be important to intervene on these risk factors in community residents, especially in elders and those with family history of stroke.
